Please Note: Never hold the ignition with a continuous flame for more than 10 seconds. Do not touch the lighter flame port after ignition. Always start with the flame adjustment wheel set as far towards the lowest (-) setting as possible. Always be sure the flame is out after each use.

What is the difference between this one and the cheaper one?

There is no difference, one is a bundle deal and one is standalone.

Since heat rises, holding a lighter upside down means melting the lighter and burning your hand. How is this to be avoided?

The lighter body is made of cast metals that can resist the flame. With the design of the lighter, when holding it upside down the flame curls out away from your fingers so they won't get burnt.
